Harmony Facilitation & PPL: Strengthening CDPAP for You
Harmony Facilitation is excited to be an approved facilitator for the Consumer Directed Personal Assistance Program (CDPAP).
We have partnered with the statewide fiscal intermediary, Public Partnerships LLC (PPL), to make sure you continue receiving the best care from your trusted personal assistants.
What This Means for You
All Consumers and their Personal Assistants must be registered with PPL in order for consumers to receive CDPAP services and PA’s to begin providing services.
Registration Information
All Consumers must be registered with PPL and have an active service authorization in order to receive CDPAP services. Personal Assistants must also be fully registered with PPL and associated to a consumer to provide CDPAP services. Registration can be done online via PPL’s system, PPL@Home.

Electronic Visit Verification
There are two EVV options for consumers and PAs:
Option 1: Mobile App
Download the Time4Care mobile application on your smartphone. The consumer and PA can each download the app separately on their phones, or the consumer can use the PA’s phone to review and approve shifts. If you have an Android device, use this link to download from Google Play:

Option 2: Using a Telephony
Use a landline or the consumer’s cell phone that is registered with PPL to call in and out through the EVV phone line at 833-278-3959, and follow the steps below.
The phone number used must match the phone number on file with PPL, regardless of whether it is a landline or a cell phone.
1. Select Option 1 to indicate that you are the caregiver
2. Enter your eight-digit date of birth
3. Enter the last four digits of your Social Security Number
4. Select your consumer from the automated list
5. Follow the prompts to clock in or out
Consumer approval of PA hours worked
In order for PAs worked hours to be processed, the consumer or DR must approve all hours worked each week by Sunday at 12 PM for it to be processed in PPL’s weekly payroll. There are 3 ways for consumers/DRs to view and approve hours worked: Time4Care, PPL@Home or via Telephony
Logging Missed Time
There are 3 ways for PAs to log missed time: Time4Care or PPL@Home
Logging missed time via the Time4Care app
To log time in Time4Care for hours that you were not able to use the app, i.e. Sabbath, please log into Time4Care and tap add past shift from the dashboard.
Logging missed time via Telephony
To log missed time via phone, please dial 833-278-3959 and follow the prompts for logging missed time
Logging missed time via PPL@Home
To log missed time via PPL@Home , log in with your username and password and navigate to the timesheet tab.
All paper timesheets must be submitted along with a paper timesheet exception form.

Completed timesheets must be submitted to PPL via one of the 3 options below:

• Mail: Public Partnerships LLC, 17 Plaza Drive, Latham, NY 12110.
All time entries, including paper timesheets, are due for submission to PPL each Saturday at 11:59 p.m. ET. Please submit only one timesheet to avoid processing delays. Paper timesheets must be submitted using PPL’s official New York CDPAP timesheet. Timesheets from other sources or programs will not be accepted.

No smartphone, tablet, or internet?
Medicaid members may qualify for a free smartphone through a federal program. Consumers can apply for this benefit at www.lifelinesupport.org.
